Checklist: Signs of deterioration — early detection

Keep an eye on changes and pass them on in time

§General condition

  • 01Does the citizen seem more tired sluggish or unmotivated than normal?
  • 02Is there new or increasing confusion?
  • 03Is mood significantly different – more sad, restless or indifferent?

  • 01Does the resident eat or drink noticeably less than usual?
  • 02Is there unintentional weight loss, e.g. loose fitting clothes?
  • 03Has urination or defecation changed from normal?

§Breathing and skin

  • 01Is breathing faster, heavier or more strained?
  • 02Is there new cough, chills, or fever sensation?
  • 03Are there new red areas, sores, or changes in the skin?

§How the mechanic diagnoses a fault systematically

  • 01Note what you observe – facts, not assumptions.
  • 02Notify the nurse or responsible person the same day.
  • 03Use your workplace's TOBS or change sheet if you have one.
  • 04Follow up at the next visit and note whether the change continues.
