Journal form: documentation of chimney sweep visits

The fields that should be included when you document a visit

Good documentation protects both the customer and yourself — it shows what has been carried out, what was found, and what the customer has been advised about. Use this template as a starting point for your own record-keeping.

§Master information

  • 01Date of visit
  • 02Customer's name and address
  • 03Type of facility (wood stove, fireplace, oil burner etc.) and chimney type
  • 04Date of most recent visit, if known

§Work performed.

  • 01Cleaning carried out: chimney, smoke ducts, hearth
  • 02Fire prevention inspection carried out: yes/no
  • 03Inspection of draught and combustion: result
  • 04Any repairs or recommendations

§Findings and follow-up

  • 01Description of any errors or fire hazard conditions
  • 02Has the customer been made aware of the finding and how?
  • 03Should an authority be notified of serious defects?
  • 04Recommended date for next visit
