Fill in one line per memory card as it is filled and copied. The form is the documentation that shows whether a card is safe to format—or if it should still be kept as the only copy.

§Rules for formatting
- 01Never format a card before at least two copies are made and verified with checksum
- 02Keep at least one copy on another drive or location than the others (3-2-1 rule)
- 03Write initials of who performed and approved each copy — accountability must be traceable
